Copies of Medical Records


You may obtain copies of your previous medical records for your own personal use for $1.00 per page. In order to get copies of your medical records, you will need to fill out a Release of Information and present a valid photo ID. Please stop by Health Information Department, and we will be happy to assist you.

Copies of your medical records that are sent directly to another physician or your insurance company are done so at no charge.

Employee Health Plan MRF Notice of Availability
Our employee health plan’s machine-readable files (MRFs) can be accessed here. These files include out-of-network allowed amounts and negotiated service rates between Wellmark and their contracted healthcare providers, as required by the Transparency in Coverage Rule. These MRFs are intended for researchers, regulators, and application developers, not for our employee health plan’s participants. For benefit and cost estimates, our employee health plan participants should use the estimator tool at www.mywellmark.com.
