Case Management
What is Case Management?
When you or your loved one are hospitalized at Horn Memorial, our Case Management team facilitates care coordination, discharge planning, utilization review, assessing needs of patients and families and conducting ongoing communication between the providers, staff, patients and families.The goals of Case Management include but are not limited to:
Helping patients and working with their insurance for coverage so they receive the most appropriate level of hospital care
Helping patients and working with their insurance for coverage so they receive the most appropriate level of hospital careCoordinating care with patients, families and the interdisciplinary care team
Providing resources and/or setting up services for after hospital discharge
